Entries will be accepted on a first-come basis. Final schedules will be mailed no later than 12/19/09.

HOW TO REGISTER

1. Mail team roster and full payment to: St. Louis Gym Centre, 315 West Pacific Ave., Webster Groves, Mo 63119
2. Submit team roster via email to: stlgc@sbcglobal.net
3. Fax team roster to: 314-968-9581

All Entries MUST include the following:
Club name, including team colors
Club address
Club phone number
Club email & fax number
Coaches' names and USAG #

Gymnast Information MUST include:
Name, Birth Date, Level & USAG # (for sanctioned only)

There is a $10.00 late fee after 11/21/09.
Refunds given with a valid doctor's note until 12/21/09. No refunds after 12/21/09.
